The prohibition of drinking ordinance has been implemented but the enforcement of this law has not been made possible so far. The failure of enforcement should be taken seriously and necessary steps should be taken to ensure that if a law is enforced, people obey it.<br/><br/>There are many examples of successful legislation around the world. For example, New Zealand formulated a policy to control tobacco use in 1890 and by 1996, tobacco use among young people had decreased by 21%. Thailand initiated a policy to control tobacco use in 1992. There was a decrease in tobacco use among adolescents aged 15 to 19, from 5.9% to 21.1%, while there was a significant decrease in tobacco use among adults from 1991 to 1996. Currently, the local government of the American state of Hawaii has started strict legislation to escape from tobacco use. By 2024, only a person who is a hundred years old will be able to legally use tobacco. This law was proposed by Dr. Parliament Richard Craigie on professional grounds.<br/><br/>In the love of cigarettes, we, the confused heads, spent a hundred years, all in vain. <br/><br/>It is necessary to raise awareness among the public about the harmful effects of tobacco use because the majority of people are unaware of its harms. It is also the duty of medical doctors to play their role in this regard and keep patients and other circle of acquaintances informed about the harmful effects of tobacco use. If a smoker can quit this habit, it will not only be the success of an individual but the entire family and circle of acquaintances will be protected from its harms.<br/>

In the 1950s, major players in the tobacco industry <br/>established their influence in the World Health Organization. They established organizations that the WHO used, and secretly provided experts to the <br/>organization who apparently worked for the WHO, but actually protected the interests of the tobacco industry. <br/>Former Swiss Health Minister Thomas Zeltner had revealed such revelations. It is astonishing that scientific experts work simultaneously for major players in the <br/>tobacco industry and the World Health Organization.An example of this is a toxicology expert who was a consultant to the WHO<br/>at a time when there was a global campaign against tobacco use. On the other hand, they were also receiving a salary from a multinational cigarette manufacturing <br/>and selling company like Philip Morris. Then there came a time when these connections became apparent to people. Despite this, <br/>they continued to work as a consultant for the World Health Organization. There are such exemplary examples where scientific experts worked against the objectives <br/>and purposes of international health organizations. When the World Health Organization runs campaigns to promote the health of people, <br/>it is estimated what will be the health of the people? <br/><br/>According to an estimate, there are a billion cigarette smokers in the world, who smoke 18 billion cigarettes daily. <br/>Experts consider it the most profitable business in the world. Eighty percent of the cigarette smokers in the world belong to lower and middle-class societies. <br/>Thirty-three million people work in tobacco fields. A large number of these workers come from extremely poor areas and classes. <br/>Laborers include small children who work for very low wages, but the views of those who struggle against child labor do not focus on these children. <br/><br/>Awareness about diseases caused by smoking is not being created. Though any joint in the body can be affected by gout but the joint at the base of the great toe has a higher chance to be affected in majority of the cases. Normal uric acid levels in blood are 3.5 to 7.2 mg/dL. A uric acid level more than this indicates hyperuricemia (high uric acid). Increased uric acid is not a disease and by itself is not dangerous. However, if excess uric acid levels lead to the formation of crystals between the joints, gout can develop. In some individuals, excess uric acid levels also lead to the formation of kidney stones.Please remember not all who have high levels of uric acid, develop gout. High level of uric acid in blood without any symptoms is termed asymptomatic hyperuricemia.<br/><br/>Signs And Symptoms Of Gout<br/>Gout symptoms occur due to the deposition of uric acid crystals in the joint spaces and or in the connective tissue of the joints. The main signs and symptoms are as follows:<br/><br/>1. Pain in joints: Gout is a very painful condition mainly affecting the metatarsal – phalangeal joint at the base of the big toe in most cases. Gout affecting big toe joint is named podagra. Pain occurs suddenly and in most cases, joint pain starts during the night and reaches maximum within 12 hrs of its beginning.<br/><br/> Joints affected<br/><br/>Gout mainly affects the joint (metatarsal–phalangeal joint) at the base of the big toe but it can affect other joints as well. The other joints that can be affected by gout include ankles, heels, knees, wrist, elbow, fingers, and shoulders.<br/><br/>Duration of a gout attack<br/><br/>One episode of gout can last from 3 days to two weeks. In between two gout attacks, no symptoms occur at all.<br/><br/>2. Swelling, redness and heat in joints<br/><br/>3. Tenderness of the joint: Joint is painful even to slightest touch and even the little contact with bedsheet seems unbearable.<br/><br/>4. Stiffness of joint and limited range of motion of joint<br/><br/>5. Tophi– Deposits of hard, painless deposits of uric acid, called tophi can appear as lumps under the skin and around the joints from long-standing increase in uric acid levels.<br/><br/>Cause And Risk Factors:<br/>The main cause of gout is an increase in uric acid levels in the blood.<br/><br/>The following factors increase risk of gout<br/><br/>1. Dietary factors: Eating food products rich in purines tend to raise the levels of uric acid in the blood. Some foods that are high in purines include meat, fish, seafood, shrimps, shellfish, beans, dried mushrooms, spinach.
